NUTRITIONAL SUPPLEMENT PROGRAM

KHAKY offers dialysis patients a variety of nutritional supplements products.* The program provides supplements at discounted prices to any patient who could benefit from nutritional intervention. Patients who qualify can receive up to $10 each month in a free product.

The process is administered through the clinic dietitian who provides an order form and recommendations to the patient. The patient selects the product they want and gives the completed order form and their payment (if applicable) to the dietitian. All forms can be found below this on the link 'Google Shared Drive.' A new patient information form must be completed for each new patient participating in the program (this is only required with the first order and also includes information required to determine if the patient qualifies for the $10 monthly allowance program). The dietitian then places all of their patients' orders via email by no later than 4pm on the first Tuesday of the month. KHAKY compiles the orders and submits a combined order to our suppliers. All orders are usually ready for pick up at the KHAKY office by the following Tuesday. Pick up locations outside of Lexington are being developed and a list of those options will then be available on the order form for the dietitian to select from. 

All orders must be paid for when picked up from KHAKY or mailed to KHAKY prior to pick up. The dietitian can pick up the orders personally or select a designated person of their choice to pick up the order on their behalf.

Emergency Financial Assistance

KHAKY provides up to $200 a year in emergency financial assistance to patients who qualify.* Covered expenses include housing, drugs and medical supplies, utilities, and public or commercial transportation.

*All patient services are administered through renal social workers and registered dietitians ONLY.
